In early 1976, Army doctors at Fort Dix noticed an unusual cluster of severe respiratory illnesses among recruits, including one sudden death, and laboratory testing revealed the culprit was a swine‑origin H1N1 influenza strain closely related to the 1918 pandemic virus. Federal health officials feared it could spark a catastrophic pandemic, so the U.S. government launched an unprecedented nationwide vaccination campaign to immunize the entire population.

But the vaccination program was halted because health officials detected a significant increase in Guillain-Barré syndrome (GBS) among people who received the vaccine. Once this risk became clear, the government suspended the vaccination campaign until further investigation.

The 1976 swine flu vaccination campaign was never resumed because the perceived threat of a pandemic disappeared, while the risks of continuing the program became clearer-making the cost-benefit balance no longer justify mass vaccination. The Fort Dix virus never spread beyond the base, and eventually officials concluded the emergency had effectively passed.

In 1976, an outbreak of the swine flu, influenza A virus subtype H1N1 at Fort Dix, New Jersey caused one death, hospitalized 13, and led to a mass immunization program. After the program began, the vaccine was associated with an increase in reports of Guillain–BarrĂ© syndrome (GBS), which can cause paralysis, respiratory arrest, and death. The immunization program was ended after approximately 43 million United States citizens had been administered the vaccine.[1]

Richard Krause, director of the National Institute of Allergy and Infectious Diseases from 1975 to 1984, writes that the government response to the swine flu outbreak was considered to be too fast.[2] The Centers for Disease Control and Prevention states, "Those who received the 1976 swine flu vaccine had a slight increased risk for developing GBS of approximately one additional case of GBS for every 100,000 people who got the swine flu vaccine."[3]

The vaccination campaign has been characterized as a failure that is partially responsible for Americans' hesitancy to trust vaccines in the present day.
