Dale Alice Martin, a 21-year-old college student, was admitted to Connecticut’s Waterbury Hospital last year for a routine broken ankle, a diagnosis that gave her parents no initial cause for concern. 

However, due to a severe lack of communication and coordination among the medical staff, she was concurrently administered overlapping, powerful doses of morphine, oxycodone, and Dilaudid. A doctor ordered the additional painkillers without instructing the nurses to stop her previous medications, leading to a catastrophic overdose that left her brain-dead just hours after her admission.
